What information will I need?
The exact information needed will vary by business. You will only need to complete the parts of the form relevant to your business. The information provided should pertain to the last calendar year, January 1 – December 31, 2023. The form asks for:
- Information about your business
- Contact information for two individuals
- Types and amounts of heating fuels you sold into or in Vermont in 2023
- Business contact information and fuel amounts for each business from which you purchased heating fuels in 2023
- Business contact information and fuel amounts for each business to which you sold heating fuels for resale in 2023
- Information about how you used heating fuels that you brought into Vermont but did not sell in 2023
- Information about heating fuel that you produced, refined, manufactured, or compounded in Vermont in 2023, if any.
