Prefiled Testimony
Commission Rule 2.213(C) states the formatting requirements for prefiled testimony. While the preferred form for prefiled testimony is question and answer format, prefiled testimony may also be filed in a narrative format. All prefiled testimony must: (1) be typed and double spaced; (2) include line numbers in the left-hand margin; and (3) be preceded by a separate page that provides a short summary of the testimony and exhibits referred to in the testimony. The Commission has developed an editable Word template that meets these formatting requirements. Forms for CPG Holders
CPG Municipal Notice Form Pursuant to Act No. 174 of the 2016 Legislative Session, the holder of a certificate of public good (CPG) or amendment to a CPG for an in-state generation facility with a capacity that is greater than 15 kilowatts shall, within 45 days of issuance, record a notice of the CPG or amended CPG, on the form prescribed by the Public Utility Commission, in the land records of each municipality in which a facility subject to the CPG is located. The CPG holder shall also submit proof of this recording to the Public Utility Commission at 112 State St., Montpelier, VT 05620-2701.
Net-Metering CPG Transfer Form For cases where the land hosting the system is sold and the transaction includes ownership of the system. (For example a homeowner sells their home and the solar panels in the back yard to a new homeowner.)
Net-Metering CPG Transfer Form for Net-Metering Systems Sold Separately from Land (For example, Solar Company A sells a system that is located on a farm to Solar Company B while ownership of the farm is not part of the transaction.)

Department of Public Service and Public Utility Commission Application Fee Form (The Department of Public Service collects an application fee from applicants for in-state generation facilities, including all net-metering facilities. Mergers & Acquisitions Notification Form
Telecommunications companies involved in mergers, transfers of control or asset sale/purchase transactions must comply with Commission Rule 7.500. As stated in that Rule, some companies only need to file a completed Notification Form with the Public Utility Commission, while others may need Commission approval. Please make sure that the Notification Form is filled out completely and includes the required attestation before filing with the Public Utility Commission. All applicants must first be registered with the Secretary of State to do business in Vermont.
Billing Aggregators Registration Form
Companies wishing to apply for certification to provide telecommunications billing aggregator services in Vermont must complete a Registration Form after reviewing the instructions.
